Fetal heartbeat: About  6 weeks(post 1st day LMP)  is the earliest you can see the heartbeat ( which  is about where you are now-depending on what day was LMP and when you had the US) . Would consider repeating in 1 week as it should be more visible. Discuss with your Ob.

By measurements: the crow-rump (head-bottom) lengths of the fetus and looking up from the chart, one can easily estimate the gestational age from fetal ultrasound imaging study.

From LMP but..: ...If the sonographic dating was obtained from measuring the "mean gestational sac size", its margin error may be wide and it may be inaccurate. If it was determined from the average of 3 measurements of a "crown rump length" by a certified experienced sonographer, it is more reliable and precise. A repeat scan in a few weeks may be valuable.

Yes: Ultrasound for pregnancy uses the same 40 week gestation time as OB to be consistent.  So, the reference date is the time of the last menstrual period, not ovulation.

Honestly, i don't know why the medical community hasn't changed this except that it has been accepted for so long.

Breast ultrasound: Diagnostic ultrasound (US) is performed to evaluate a specific breast symptom (e.g., lump) and may be performed at any age.  Screening US is performed to detect cancer in women with no breast symptoms, and should be performed in combination with annual mammograms (MG) beginning at age 40. MG still remains very important for detection of some small breast cancers that can easily be missed by US.

kidneys have slight decrease in size. right 8.1cm, left 8.9cm. no hydronephrosis or nephrolithiasis. what does this indicate? 38 yr female.  ultrasound impression. age, or ckd, or kf?

Not much: Since kidney size is not an indicator of most kidney problems I would not be concerned. You may be dehydrated or more hydrated on certain days. The kidneys pass through a lot of fluids.
