Colon cancer: She should consult an oncologist and surgeon as soon as safe to do so.  Start with telephone. Whoever diagnosed this for her should be where you start and he/she can refer your aunt for definitive medical and surgical treatments.

Grade 1 is a good: Sign, but nothing is guaranteed.  Need to further understand the liver lesion as well.

Treatable: In general, stage 4 (metastatic = spread) colon cancer is incurable. That said, a small % of people may have long term survival or maybe even cure.  Important predictive and prognostic tests include kras (for cetuximab) and braf (+ = poor response to traditional chemotherapy). New drugs are coming out for this disease.

Surgery, chemo: Depending on the stage, an operation is usually necessary to remove the diseased portion of the colon along with its nearby lymph nodes.  Then, depending on if the cancer has spread to those lymph nodes impacts whether you will also need chemotherapy. If the cancer has already spread beyond the colon, sometimes you start with chemo.

It depends.: It depends on how early the stage is.  This is staged by the duke's classification system.  Stage a has >90% 5 year survival where tumor is confined to the mucosa and submucosa.  Stage b is 70-85% goes up to the muscularis, serosa, and pericolonic fat.  Stage c has 35-65% 5 year survival with lymph node involvement.  Stage d has 5% 5 year survival with distant metastasis to lung, bone, liver, etc.

Possibly: A number of different adenoCa's can spread to liver. Colonoscopy followed by PET/CAT may be needed to determine origin of the metastatic lesion.  Once primary is addressed, the liver can be handled since lesions of liver do not spread out of liver but primary's can spread to other sites.  Liver then approached by resection, microwave ablation or chemoembolization.

Need more info: The code for colon cancers is 153._. 153.9 is for colon cancer of unspecified site. There is a separate group of codes for metastasis and the site. The number after 153 is for the site of the primary tumor. 
I can't answer the cpt code question, because  there is not enough information.

Very poor: Metastatic cancer to bone portends a fatal outcome unless it is one of those uncommon cancers that is curable with chemotherapy. However, the time frame is famously unpredictable. Local radiation to the bone lesions may help.
