can you please tell me about the active range of motion and passive range of motion tests for a torn acl?

Exam for ACL-"ROM": Passive motion is movement caused by some outside force. Active motion is movement you generate yourself.  These are not specific tests for an acl tear.  The acl provides stability to a knee, so if there is a significant tear, there will be too much motion of one leg bone sliding over the other bone. This is "translation" of the femur on the tibia vs flexion (bending) or extension (straightening).

Motion capacity: Passive motion refers to the range of motion the spine is capable of when induced by out side forces. Active motion is what it can do when your own muscles are activated.
Injury or spasm of muscles can reduce both active rom and passive rom.

Pain : Pain in the muscles can be caused by nerve root irritation, fatigue, electrolyte imbalance (sodium or potassium), and sometimes a cause cannot be found. Rest and cool or warm packs often helps muscle pain.  The patient should be sure that they are well hydrated.   If a patient had difficulty walking, weakness, numbness, or other neurologic complaints then then they should have a medical evaluation.

: 5/5 is not usually used for range of motion but used for strength documentation on an examination of a patient. 5/5 means the strength is normal. Whereas 4/5 would mean 80% of normal strength.

: A normal range of motion indicates a healthy joint and muscles. You did not state why the range of motion was limited? What was the treatment? Is the range of motion with the normal range or excessive range?

Muscle strain: The cause of a soft tissue injury or pain coming from an injured nerve will often not show up on a plain xray. Multiple syndromes of soft tissue injury are possible, especially if there is a history of rotator cuff injury in past. Nerve impingement syndromes also may explain this type of pain. It may be possible to narrow down the cause with a detailed physical exam. See your doctor if it persists.

i have shoulder pain that feels better when i exercise but the range of motion is limited. what could it be and should i stop excersing.

Gentle exercise OK: Maintenance of range of motion is always a laudible goal as long as your pain is not made appreciably worse. Orthopaedic surgeon evaluatin will efficiently elucidate the underlying pathology. More common possibilities include: adhesive capsulitis, subacromial impingement (bursitis), a partial thickness rotator cuff tear, and/or underlying mild to moderate shoulder joint arthritis.

when trying to make sure i have full range of motion in my neck, do i measure to the point i can move it, or to where it starts to hurt?

It depends: For your therapy where it starts to hurt would be most convenient.  Then you could also measure the extent of movement so that would be your goal to attain from your therapy

Yes: No harm in getting an x-ray to make sure.
