Biochemical proof: Hi. Before any imaging of the pancreas is done, biochemical proof of fasting insulin-mediated pathological hypoglycemia should be established. Has that been done? If so, EUS is good, but still not as good as intraoperative US. If endogenous insulin-mediated pathological hypoglycemia is proven, the problem lies in the pancreas, and surgery is warranted. We need more info. Most insulinomas not CA.

Back pain/stomache p: If pain is on the RT side it could be kidney or liver. If on the LT it could be the LT kidney  This could be stones or an infection  You should have some lab tests done such as CBC,CMP,,urinalysis and possibly some XRays.   Also if you are in child's bearing age. And not on birth control this also needs to be checked

Biliary trouble: The first condition that comes to mind is blockage of the normal bile drainage into the intestines due to either gall stones stuck in the common duct or a pancreatic or papillary tumor causing billirrubin to rise in the blood and showing up in the urine. The presence of pain favors gall stones. Jaundice would be present.
