MRI: Breast MRI is the definitive imaging technique.  Ultrasound can also be useful.  Mammogram usually less helpful because it can't detect intracapsular rupture and can only sometimes detect extracapsular rupture/leak.

No I have not: But a patient wanting a larger breast implant is not uncommon.  It is best to discuss this with the surgeon who placed the original implants.  He would be the best to recommend whether he thinks you would be a candidate for this or not.  It may require tissue expanders to stretch the skin more before going to a larger implant.

Unusual: Silicone buttock implants should not be associated with any vaginal bleeding unless the stress of the surgery disrupted your menstrual cycle. Would consult with your plastic surgeon and gynecologist.

Nerve impingement?: This is most likely located on the outer part of the implant and may be due to nerve impingement by the implant. This may due to nerve preservation in an effort to maintain nipple sensation, occasionally breast implant exercises may help. Although capsular contracture may present with pain, it tends to be progressive. Nsaids may help. Discuss with your surgeon.

Breast surgery ; BA: No it should not. Your surgeon should be careful not puncture your implant.

No: A fibroadenoma does not need to be removed unless it is painful, growing or over 3 cm. There is a slight risk of implant rupture but that would be very unlikely to occur.
