Condom efficacy: Efficacy rate with condoms are 98% when used consistently & properly. Even if you did not ejaculate - semen may be released via pre-ejaculate.

Broken Condom: The only advice I have in this situation is to share this information with your sexual partner so that if there is any disease risk it can be assessed. And, if there is pregnancy risk then this too can be dealt with the morning after pill. Good communication is the best advice i can recommend.

Pretty good: The pull out is not a method. I call it baby roulette. When a condom is applied before any sexual contact starts, is the proper size, is never re-used, it is an effective method.
