The pattern suggests a skin reaction to an agent involved in the activity. Does this include a condom that ne may react to or a lubricant? Those are the type of things that I would look into.

Sometimes due to irritation sometimes to allergy. Try with a cndom or a thick layer of KY jelly or other OTC lubricant on the penis.
