Ive had on and off pain in genital area for three years now. can both chronic pelvic pain syndrome and urethritis come and go over years? basically the pain comes for weeks then goes away and comes back?

Yes, recurrent episodes of pelvic pain and dysuria are the hallmarks of this syndrome. I wonder if you have been treated with an alpha blocker. If not, you should see a urologist for help.

Urethritis is almost always an STD and rarely if ever causes intermittent symptoms for several years. CPPS does so very commonly.
