Usually a torsion causes pain. If there had been torsion, pain then no pain would be worrisome. The best way to “see” what is going on is with a scrotal ultrasound. Don’t wait.

Testicular torsion is associated with sudden onset of severe pain. Torsed testis is usually higher & larger than other testis, but also very tender. Go immediately to hospital emergency room if torsion is suspected because testis is at risk of permanent damage after 4 - 6 hours because blood supply is cut off. Risk of loss of testis after 6 hours. Not torsion if there is no pain. ? tumor if hard.
