Unsafe sex: It doesn't sound like you are trying to have safe sex. PrEP only protects against HIV, so you need to use condoms to protect against other STDs. If taken religiously, PrEP can reduce risk of HIV from sex by 99%.

Yes: Any new bleeding in pregnancy should be reported to your doctor/cnm. The cervix is very friable (bleeds easily) in pregnancy. Because of this, many women experience spotting after intercourse. I would not qualify this as normal, but it is very common and most times not worrisome.

No: The definition of "work" with birth control pills is vague, it usually means not getting pregnant. For them to not work can be because they weren't taken every day or other meds were taken that decrease their effectiveness, such as antibiotics. This is not genetic. If "not working" was having side effects, like mood changes, to some types of hormones, that may or may not happen to family members.
