If one breaks, it would have to be a tear that makes a big enough hole to be seen. Otherwise, nobody would notice it broke. The latex or nonlatex material is elastic, so a condom that's not fully ripped should still be on the penis.

Pinholes & pinworms: A small pinhole in the condom is the most common 'break" that occurs leading to a "pinworm" (sperm) to escape and make the "journey to paradise" to meet the egg. The second most common condom "break" cause, is when the condom is placed on the erect penis without allowing enough space at the end to capture the ejaculate.
