I have previously had a blood clot due to birth control pills. my dermatologist is aware of this but she still prescribed me a testosterone surpresent called spironolactone. can i take it? or is it putting me at a greater risk of another blood clot?

Indirectly: this can raise your relative estrogen level, but no direct effect on blood clotting. It may be worth your time to discuss this with your dermatologist, and maybe see a hematologist to best understand your risks.
