Extremely unlikely: It is almost impossible for her to get pregnant this way. There would be an extremely tiny chance of pregnancy if the crotch area of her underwear touched a very wet part of the napkin and then that wet area pushed up very closely to her vagina. Even then, though, it's VERY very unlikely.
