Keep the clothes on: No worries. Just keep the clothes on, because the porous fabric will prevent an airtight seal. Thicker fabric such as denim is even more protective. Air embolism is a risk if a person's mouth and lips seals off another person's vulvo-vaginal opening, and then he blows... that would be a bad thing to do.
