Unlikely, but...: is this the only time this happened? breakthrough pregnancy while on OCP are uncommon. Pregnancy tests are sensitive enough to catch an early positive if in doubt. Embryo implantation can cause "early mild bleeding" usually around the 21st day of the cycle.
