No: No need to feel awkward. From a clinician's standpoint, being sexually active means one is engaging in sexual activities with another person or persons.

Game of words: The framework by which many view sexual activity usually implies partner sex. However, self sex is a form of sexual expression that is considered normal and I would consider it sexual activity. If the doc is asking you about sexual activity it will usually apply to partner contact.The other is assumed to happen anyway.
