Infection.: YOu need to see the doc tonight or tomorrow. Have you had your penis somewhere you should not have?
The other question is have you been circumcised?
This could be a balanitis which is an inflammation of the glans and you may need to take antibiotics and then later be circumcised if the foreskin is infected . antibiotics for now is a concern and need to be evaluated very soon.

See remark: You must be uncircumcised.The foreskin is tight and acting as a tight rubber band on head of penis.This results in swelling ,redness,possible inflammation.
The remedy : Sit in a warm tub,or dip penis in warm jar,gently try pulling foreskin forward.Sometimes you may have to use a dab of antibiotic ointment for lubrication.If feasible,see an MD,preferably a urologist.
