Many possible things: In the first 1-3 mos, spotting/irregular bleeding is quite common, and can occur even with perfect pill use. It doesn't mean there's a a problem. But if you've been on the pill for awhile, there are many possible causes of bleeding-- from infections, even stds, to polyps of the uterus or cervix, to fibroids, to missed pills, or other causes. See your doctor.
