Neither: The best treatment for a chronic anal fissure is fiber, a good pain regime, and possible chemical or surgical sphincterotomy to eliminate spincter spasm and allow healing.

Consider surgery: If you have a chronic anal Fissure, then it is obviously not responding to conservative treatment. Chronic fissures need to be treated surgically in order to resolve the problem.
