Trichomonas: A strong fishy odor is classically associated with trichomonas, an std. It could be associated with other issues, or just your normal flora; if this is a change, i would get evaluated by your doctor and see if treatment is warranted.

Yes: Foul smelling discharge needs to be evaluated. The most common cause is a nonsexually transmitted overgrowth of bacteria called bacterial vaginosis. Testing for other sexually transmitted disease may need to be done as well.
