Many reasons.: The most common reason is inflammation of the bladder or urethra; usually caused by an infection such as a UTI or std. Specific patients may suffer from stones or tumors in the bladder. Problems with the urethra (diverticulum, stricture), or vulvar pathology cause pain. Some neurological conditions can cause dysfunction of the sphincter, causing painful spasms while voiding. Seek evaluation.
