Not common: Some women and most men ejaculate with an orgasm which can cause "a wet mess" that requires more laundry. If the woman has a uterine infection then an orgasm can cause pain. Hematospermia (blood in semen) is harmless but less than wonderful. Are you trying to talk your self out of having an orgasm? Relax and enjoy it, it is natural!
