Infection: Infection( viral or bacterial) is the most common cause. The infection may not be due to an STD. Brucellosis can also cause it,(per wikipedia). If you've had trauma to the urethra, it can do it, tool. Sometimes an infection will not be evident on routing testing. Many docs would try an round of antibiotics even if cultures negative. If you aren't getting answers from your doc, get 2nd opinion
