Phimosis or tight: frenulum or both. Best advice is to consult a urologist. Treatment for phimosis (Narrow foreskin opening) is either applications of prescription strength steroid cream to tight are of foreskin or circumcision or prepucioplasty. Treatment for tight frenulum is minor surgical incision transversely then sutured vertically.
