Generally Not but...: There are reports of significant mdma (i.e. Ecstacy) use with documented brain changes seen on mri. Those patients also had significant neurologic symptoms. In general, it is not likely to see major changes with recreational use. But, again this depends on how "recreational" is defined. Please don't do drugs.

Also: Some drugs, classically cocaine, can present as hemorrhages on ct or mri.
