Determine cause: Hives are caused by the release of a substance called histamine, which causes the characteristic "wheal and flare" skin reaction. Allergy tests are almost always negative, in my experience. If long-acting antihistamines have not worked, your hives may be due to an internal trigger, and your doctor will need to run some tests to find it.

Hive: One hive is a rare occurence by itself, having it last five days is also uncommon, you need to have your rash examnpined tomdetermine what it is, it may not be a hive.
