Many things: Many things are on the differential. Cough can be worse when lying flat with both acid reflux, and post-nasal drip/allergies. Also, sleep apnea can cause gasping/coughing/choking sensations, especially if it happens as you are transitioning from wake to sleep with collapse of the airway. See your doctor and he/she can take a good history and further testing can be ordered if needed. Good luck!
