It depends: Any abdominal trauma has the potential to injure your spleen or other viscera, and cause internal bleeding. That said, it generally takes a great deal of force to cause internal bleeding, such as from a car wreck or serious altercation. However, if you have a condition that gives you splenomegaly, it is much easier to be injured. This is a common problem with infectious mononucleosis.
