Surgery likely: Sounds like you are passed due for surgery.Studies have indicated that if your symptoms have not resolved after one year of conservative treatment, you are not likely to improved without surgery. Check with a spine surgeon for full evaluation.

Diskectomy: If pain management and physical therapy have not given good relief and it has been four years would suggest a consultation with a neurosurgeon to discuss possible diskectomy.
