Hard to say : Go see an ear, nose and throat doctor. The two most likely things are a clot from a nose bleed or a small polyp. The ENT can look into your nose and find out for certain.

Depends: Despite it's appearance, it is still likely mucus (/snot), with a mix of blood and inhaled dirt. You won't 'sneeze your brains out, ' so it's not that. Tiny chance could be a nasal polyp that was jettisoned. If isolated occurrence, would not worry. If recurs, see your dr or an ENT for nasal-pharyngeal exam to be sure nothing else going on.
