Does bleeding and cramping ever not mean miscarriage?

Yes: Most of the time moderate to heavy bleeding and cramping in early pregnancy means a miscarriage. But some women with bleeding and cramping don't miscarry. I will often check either a blood pregnancy test or an ultrasound to tell if there is still a viable pregnancy or if this is a miscarriage situation.
