Hiatal hernia: This cannot be felt on physical exam so some sort of test is needed to diagnose, ex: x-ray swallow or upper GI endoscopy. This should be diagnosed and properly treated because symptoms of hh are similar to other much more dangerous conditions, such as malignancy.

Impossible: A hiatal hernia can cause reflux, heartburn, difficulty breathing(if large) after eating. The problem is that all these and other symptoms can be caused by othe conditions. The only way to know for sure is through a barium swallow and or endoscopy.
