I can...: The method to truly tell is based on history and examination. Have a physician check if you are concerned about appendicitis. If there is not enough evidence to tell, then lab work and a ct scan might be in order.

Usually: As a groin pull would be muscle soreness with walking or straining & otherwise eating & feeling well. Appendicitis is usually mid abdominal pain that then moves into the right lower abdomen with nausea, ill feeling & sometimes vomiting with fever. See a doctor asap if you are uncertain.

Pulled groin: Appendicitis will progress in pain over time. Appendicitis may hurt more in the belly and your body overall. A retro peritoneal appendicitis may feel like a pulled groin muscle. For appendicitis you will need medical assistance.
