Repetitive extension: Multiple causes can lead to extensor carpi ulnaris tendon pathology. The mechanism to "tear" would be loading the tendon by extension and radial deviation repetitively or a significant lift resisting this motion. Secondly inflammatory processes like rheumatoid arthritis and gout can weaken the tendon prior to tear. Finally an old fracture or RA with prominent ulna can compromise the ecu tendon.
