Unlikely : L4 is in low back lumbar area. This would not affect the neck. You need to see your physician and then get this clarified.

No: I'm not sure if i understand correctly (whether there was a typo) but an L4 lumbar nerve issue should not have any bearing on neck symptoms. To clarify however - what is the thecal sac "lesion?".
