Low: With correct usage condoms are roughly 98% effective, the failure rate is higher if not used correctly.

Possible: If the condom breaks, then your chances are as a good as if there was no condom. If you were ovulating at the time, your chances are even greater. Check pregnancy test.

Not clear: Failure rate of the withdrawal method (pull-out method) is 15-30%; averaging about 25% (1 in 4). However, I am not aware of any studies evaluating it during ovulation; i suspect it might be higher. While the withdrawal method may be successful with the right partner, it takes a responsible couple to utilize this method to make it an effective choice. Consider avoiding sex while ovulating also.

Possible: As long as you are having sex you can get pregnant. Of course contraception decreases the chances but it is still possible. Abstinence is only 100% way of not getting pregnant.

Take plan B: If it happened within the last 72 hours and you do not wish to get pregnant, take plan B. It is not feasible to give you odds of getting pregnant. If you do not wish to be pregnant, use contraception all the time, every time. You may consider implanted contraceptive, or IUD. Practice safe sex if you have sex. Get HPV vaccine

As long as no sperm: Leaked out of the condom then no, if sperm leaked out or around it then yes.
