Nothing: Toothpaste contains certain ingredients like baking soda, hydrogen peroxide, alcohol, menthol, essential oils, triclosan that may cause pimples to dry out. However, there are no ingredients in toothpaste that make this method more effective than conventional treatments. Toothpaste actually irritates the skin and causes redness and peeling.

Toothpaste is drying: Toothpaste is well established as a home remedy for acne. Unfortunately, other than being drying for active pimples, it is not particularly helpful. Try using over the counter products containing benzoyl peroxide and/or salicylic acid instead. If the acne persists, see your dermatologist.

DO NOT USE: Toothpaste is for teeth not acne. See your doctor to treat your acne. Toothpaste can cause a chemical burn to acne skin.

Short answer: It can irritate/burn the top layers of skin so bad you have permanent scaring. Save it for your teeth. If you use it and it happens don't say nobody warned you.
