Yes: Varicoceles occupy a portion of the scrotum (sac) and can compression the testis and decrease fertility, libido, etc. This can be due to poor temperature regulation and restriction of blood flow to the testis.

Apnea: I regularly check free and total testerone levels on anyone with sleep apnea. My experience has been that in sleep apnea patients, low t is inordinately prevalent, even in men in their 20's.
