Levaquin (levofloxacin): Yes, Levaquin (levofloxacin) can cause tendon rupture, although, the incidence of it is small and I am yet to see a case of it in my practice. Methypred doesn't increase the risk of such. This is a choice to make and you could have asked the doc for alternatives.

Monitor: Tendon rupture or tendonitis is a rare but well-established side effect of levaquin (levofloxacin). The vast majority of people tolerate even several week courses of Levaquin (levofloxacin) without problems. Contact your surgeon if you experience any tendon or joint pains.

Concern is low: Even when taking Levaquin (levofloxacin) and steroids together the risk of tendon rupture is very small. The risk increases as you get older. Almost all of the tendon ruptures have occurred in people older than you (70ish). Unfortunately, the combination of Levaquin (levofloxacin) and steroids is a very effective treatment for sinusitis.
