Anemia and pain: Anemia can be caused by multiple underlying pathologies (ie. Iron deficiency, vit B12 deficiency, lead poisoning, chronic disease, blood cell dysfunction, sickle cell...). Some of these can lead to painful joints. If you have anemia the best thing to do is to find the source of it. You should also have your doctor perform a shoulder physical exam to rule-out internal pathologies. Hope that helps!

Which shoulder?: Just a couple of additional, albeit esoteric thoughts, neither of which is likely to be applicable to you. First, when anemia reflects chronic hemolysis, pt is at risk for pigment stones and gall bladder disease. Cholecystitis can present with referred right should pain. If however, a pt rapidly developed anemia that impaired myocardial perfusion, left shoulder pain could reflect ischemia.
