It's not creatinine: ...That dictates need for dialysis; usually it's electrolytes such as potassium that build up to toxic levels because the kidney cannot get rid of them.

Not clear: I'm not sure on the units in your value or if there is a decimal missing. A creatinine of 4.80 mg/dl is abnormal and requires investigation. A creatinine of 480 micromol/l is also elevated and requires evaluation.
