See my answer: Pain during the period in common is this new event ? Have you not had pain during periods? When were you checked by a gynecologist? If you don't have one , a family doctor? You can seek help at planned parenthood if you don't have a doctor.

See GYN: At 46yo there are several possible causes of dysmennorhea (painful menstrual cramps). See a gynecologist for an exam and possible imaging studies of your ovaries and pelvic area to get started on the evaluation. Good luck.
