Need exam: At 18, could be "growing pains" or a common condition such as chondromalacia patellae (also known as cmp), which is inflammation of the underside of the patella. While it often affects young individuals engaged in active sports, it can also affect older individuals too. Overall. Impossible to definitively diagnose you over the computer of course, so see a dr for an eval. Good luck.

Probably instability: Probably related to joint instability. Exercises to strengthen quadriceps muscles and assuring good foot mechanics generally helps.
