Yes: When a patient is suspected of have any type of infection, a physician should test for it and treat the patient appropriately. The common sense answer here is that if a patient seeks medical care from a physician, then by asking for help, the patient wants to be diagnosed and treated. A test for an std may be only several tests performed to be certain of the diagnosis and proper treatment.

Yes and no: If you present for care with a physician you must provide written consent for them to evaluate and treat you. That consent form will set the parameters for the evaluation. If testing for a STD is part of their routine workup they do not have to ask again for specific permission, it is implied when you authorize the evaluation. Only drug testing requires a separate consent.
