COPD more broad: COPD is a functional diagnosis, while emphysema is a diagnosis you make on a ct scan or pathology slide. COPD includes both emphysema and chronic bronchitis. Emphysema is destruction of lung tissue most often due to smoking. Chronic bronchitis is a chronic, productive cough, also most often due to smoking. The end result of both is the same: inability to blow all the air out of the lungs.
