Rule out angina: The main thing to consider (and hopefully exclude) is if the nature of the pain or tightness is related to the heart. Sometimes the coronary arteries can spasm, or be blocked leading to angina...or even a heart attack. Other possibilities to consider include,: asthma, esophageal spasm possibly related to reflux pulmonary embolism, pneumothorax, pneumonia, copd, anxiety.,..and other things.
