Needs examined: Could be local problem, or systemic. Skin infection or herpes--cannot say based on 2 lines via internet w/o exam. He needs to see his dr, or an urgent care / 'std clinic' dr (better anonymity if concerned), or a urologist for proper exam, testing, and treatment. Good luck!
