Number of things: This includes period issues or ovarian issues and any abnormal ob/gyn issues should be worked up. However, pain in low back on the sides can be anything from muscle tightness to sacroiliac dysfunction, kidney stones or irritated nerves. You should have a physical exam to differentiate between the type and causes of the pain to make sure it is not something internal you might be missing.
