He should be!: It's not unusual to worry about each other, but yes - he has at least 1 problem - maybe a few. That swelling is a early symptom of liver cirrhosis. It is quite serious and he needs to see a doctor as soon as possible. Since this is so important, the issue of binge drinking must be put aside for the immediate moment, but it's a very dangerous thing. Moderate, regular drinking is much, much safer.

Yes: First concern is what it does to the liver. The swelling is because alcohol is a vasodialator which opens the blood vessels and gravity pulls it down thus swelling. Cut back the alcohol.
