Surgery: Presumably you have either an ac joint dislocation (shoulder end of collar bone) or a sternoclavicular joint dislocation (chest end). Either way, i'm not aware of any nonsurgical method of putting it back in place. The vast majority of cases actually don't need to be put back in, because patients typically don't have much if any pain, and they function fine even if it may look a little unusual.
